Step 1: REGISTER WITH US AS YOUR QUARANTINE FACILITY!

Department of Agriculture Guam (DOAG) will require you to register with a quarantine facility if your pet is originating from a NON Rabies free area. Regardless of your pet’s status meaning service animal, emotional support animals, and or working dogs this requirement is still applicable to your pet’s entry if you are coming from a zone with that status.

Step 2. GET IN TOUCH WITH DEPARTMENT OF AGRICULTURE IF YOU HAVEN’T DONE SO ALREADY.

You should communicate with both your quarantine facility and the Department of Agriculture.

Animal entry permits are applied with Department of Agriculture Guam.

Step 3. SUBMIT ALL REQUIRED DOCUMENTS

All documents MUST be submitted to both parties. It will be the owners responsibility to make sure of this.

Travel Health Certificate

FAVN Report (Passing score ≥ 1.0 & the results must be sent to DOAG directly from the lab)

Current Rabies Certificate

Prior Rabies Certificate

Proof of Bordetella (For dogs)

Proof of Leptospirosis (For dogs)

Proof of DHPP or DAPP (For dogs)

Proof of FVRCP (For cats)

Animal Entry Permit

You must also include your confirmed flight itinerary and a photo of your pet.

If your flight schedule changes you MUST inform us of this change to avoid additional fees and charges. Please be advised that we have to schedule customs hours prior to your arrival and will need to schedule the licensed Veterinarian to be the airport to clear your pet.

Your reservation deposit fee will be valid for 6 months for any APPROVED cancellations.
